Optomap detects occluded artery

Hattiesburg Eye Clinic’s Dr. David Richardson remembers clearly what he was thinking when he first saw a picture of his eye taken last May with Optomap, a new type of retinal imaging camera.

“I told the company representative, there must be some mistake,” Richardson said. “This has to be the image of another patient — it can’t be mine.”

Jim Brennan with Optos, which provides the Optomap system, was demonstrating the new device to the clinic’s physicians and was using Richardson as a test subject. The image, which captured a wide panorama of the retina on the back wall of his eye, indicated an abnormality

“Viewing a patient’s retina primarily helps us diagnose vision problems, but it can also indicate problems elsewhere in the body,” Richardson said. “And that’s what I saw when I looked at the image of my eye — a possible blood vessel blockage somewhere else in my body.”

Brennan assured him the demonstration camera had never been used before and that his eye was the first one imaged by that particular unit. Alarmed, he called Dr. Robbie Robbins, a cardiovascular surgeon who set up a cardiac CT scan a couple days later on Friday. After undergoing the procedure, Richardson went home. A few hours later, Robbins called him.

“He asked me what I was doing,” Richardson recalled. “He then said, ‘Don’t do anything strenuous the rest of the weekend — I have a heart catheterization set up for you early next week.’ ”

The CT scan had revealed an occlusion of the left anterior descending artery of the heart, known commonly as the “widowmaker” artery. Heart attacks involving it are extremely dangerous and life-threatening.A few days after the surgery, he called Brennan with Optos. “I thanked him for bringing the camera that day. I believe God used that demonstration to save my life.”

The eye’s retina is a thin layer of cells that line the back of the eye and make up nearly 65 percent of the eye’s surface. It converts light into impulses and sends them to the optic nerve, which transmits them to the brain. Because of its extensive surface and network of blood vessels and nerve fibers, it’s an excellent indicator for potential vision problems such as macular degeneration, glaucoma or retinal detachment, all of which can lead to blindness. But it may also contain signs of other problems in the body such as increased blood pressure, diabetes and heart disease.

“We actually see quite a number of cases of general disease during our eye exams. I would estimate that we refer 10 to 20 percent of our patients to other physicians to check on abnormalities we’ve found,” Richardson said.

Your Eyes are more vulnerable than your skin

The effects of sun damage to the eyes is cumulative — so while the cells can heal, lasting damage can slowly build up.

This means that years of seemingly harmless episodes of UV exposure such as visits to the beach without the right protection could lead to serious problems long term, such as cataracts (where the lens becomes cloudy), age-related macular degeneration (damage to the light-sensitive area at the back of the eye), climatic droplet keratopathy (where the cornea becomes cloudy), pterygium (pink, fleshy growths on the eye white that can alter the eye curvature and distort vision) and could, more rarely, contribute to eye cancer.

Sunrise and sunset most dangerous for eyes

But this natural protection is limited if rays are coming from in front of the eyes — which makes sunrise and sunset bad news.

The standard advice for protecting the skin is to avoid the sun when it’s at its highest point in the sky, around noon

However, the eyes are naturally shaded by the brow ridge at noon. It’s actually when the sun is rising and setting that it poses the highest risk to eyes, says Russell Peake, as this is when the sun is lined up perfectly for its rays go straight into the eyes.

And don’t presume that if you are looking away from the sun that your eyes are safe. Even if you are reading on a device or using your phone – any screen would reflect sunlight, potentially back into your eyes, says Russell Peake

Children and blue eyes most at risk

As you get older, the lens becomes more yellow and so naturally absorbs more UV which protects the retina. ‘This is why it’s important to encourage children to wear sunglasses,’ explains Nigel Best, an optometrist. ‘Because the effects of UV are cumulative, exposure when young can result in eye problems in later life.’

By the time you hit later in life damage can already have occurred, says Mr Anderson. People with blue eyes are more at risk as they have less protective melanin pigment in their iris.This makes them slightly more susceptible to damage.

Chris Carter, group sales and marketing director at mark ‘ennovy, said all of the big contact lens supplier have focused their attention on what he describes as the ‘average eye’ and this risked some contact lens fitters making compromises.

Once you move outside the needs of the average eye fitters have to tell patients they can’t wear contact lenses or fit a ‘good enough’ lens. ‘Offering the ‘good enough’ is shaky ground,’ he said. At a time when practices needed to differentiate being able to offer contact lenses which met all wearers’ needs was a valid route to take.

‘It is a premium product and a lot goes into designing a custom contact lens for a patient and we make no apology for that. It’s a little more expensive but it comes back to how much is your eyesight worth. If the product is right and meets a need the price become irrelevant.’

Optomap Patient Stories – Nakivia’s story

patientstories_nakivia_optomapIn 2009, Nakivia visited Dr. Andrew Kraft’s office in Enid, OK. During her visit she received information about Optomap and had her first screening that day. In May of 2013, she decided to have another Optomap done to see if there had been any changes in her eye pathology. The comparison between her 2013 results and previous screenings showed that her optic nerve was bending and there was hemorrhaging of her blood vessels. Within 4 hours of receiving her Optomap at what was supposed to be a routine exam, she saw Dr. Barry Pollard, and received a specialized MRI.

The results of this MRI showed that Nakivia had a substantial brain tumor that was causing the optic nerve damage and hemorrhaging. Nakivia received her surgery and eight months later, she is not only alive but her vision has improved by more than 50% without corrective lenses. Nakivia’s doctors suggested that without the immediate attention based on the results from the optomap exam, she would have most likely had a stroke or seizure within a few months.
